WHAT YOU'LL DO

As a Weekend Start of Care RN, you'll play an important role in helping patients begin their home health journey.

You'll:

• Complete Start of Care (SOC) assessments and OASIS documentation


• Evaluate patients and develop individualized plans of care


• Coordinate care with physicians and the interdisciplinary team


• Ensure patients receive a smooth and successful transition to home health services


• Provide education and support to patients and their families


• Help patients achieve their healthcare goals and remain safely at home

WHAT WE'RE LOOKING FOR

• Current, active Pennsylvania RN License


• Previous Home Health experience REQUIRED


• Strong OASIS and Start of Care assessment experience


• Excellent clinical judgment and assessment skills


• Independent, dependable, and organized


• Valid driver's license and reliable transportation
