Hours:

Regular Full-time, Overnight Weekend Friday, Saturday, and Sunday  

Shifts:

On-Call after hours 4:30pm to 8:00am Friday, Saturday, and Sunday  

Salary:

Guaranteed 30 hours of pay   

The Weekend Overnight On-Call Registered Nurse (RN) provides triage support and PRN after hours visits to home health and hospice patients. Initial calls handled by on-call clinical triage company prior to escalating to the Weekend Overnight On-Call Registered Nurse (RN). Your Responsibilities for the Weekend Overnight On-Call Registered Nurse (RN):

• Ability to triage all agency patient (home health and hospice) related phone calls
• Complete all patient communication related documentation utilizing patient EMR
• Collaborate and communicate effectively with other team members/program managers
• Provide home health and hospice visits in patient homes and facilities
• Travel throughout entire service area
• Other duties as assigned

Qualifications:

• Current NH Registered Nurse (RN) license
• Minimum 2 years of health care experience required
• Home Care experience is preferred but not required; we will train the right candidate
• Telephone triage experience preferred

Additional requirements include reliable transportation, valid driver’s license, automobile insurance, current physical exam, TB testing, and required vaccinations.
