Job Description - Wellness Director

Job Summary:

The Wellness Director will assist the Administrator in maintaining the standards set forth in the Cumberland Village Policy & Procedures and the SC Department of Public Health regulations for the care, watchful oversight, and well-being of our residents. The Wellness Director will ensure the provision of quality resident care and promote the residents’ health and wellness through regular visits with each resident, resident assessment, and ongoing communication with the resident, the resident’s representative(s), and the resident’s care team. The Wellness Director is a part of the management team of the community.

Supervisory Responsibilities:

  • Supervises care given by nonprofessional staff and provides instruction as needed.

Duties/Responsibilities:

Resident Health and Wellness 

  • Meet with each resident on a regular basis to provide health and wellness checks and to answer any health-related questions. Provide a semi-annual assessment on each AL resident. 
  • Review all resident information and complete an assessment of the resident prior to move-in, when changes in the resident’s condition occur, and as required by Cumberland Hills policy and state regulations.
  • Review the resident’s medical report prior to move-in to ensure that the facility can provide for the resident’s individual needs and regulatory requirements have been fulfilled. 
  • Develop and maintain an up-to-date individualized and personal Negotiated Service Plan for each resident. 
  • Order medications and oversee the resident’s medication schedule for residents receiving medication assistance services in compliance with state, federal, and Cumberland Hills guidelines. Ensure proper documentation and procedures for physician’s orders, medication assistance, injuries, incident reporting, and resident status changes or levels of care. 
  • If onsite, act as a first responder in the event of an emergency.  Maintain ongoing communication with the resident, resident’s family, staff, and medical providers regarding the resident’s medical needs. Coordinate additional health services the resident may need. 
  • Maintain positive relationships with Home Health Agencies, Hospice Agencies, Vendors, Physicians, and other Resident Services Providers. 
  • Assist in coordinating residents’ dietary needs with the Food Service Director. 
  • Be on call, as needed, for medical questions and resident emergencies as scheduled.

General Management 

  • Assist in the mentoring and training of all resident care employees in accordance with the Cumberland Hills policies, in-service training programs, and state and federal regulations to ensure regulatory compliance. 
  • Ensure that all infection control guidelines are met. 
  • Maintain residents’ wellness files, ongoing assessments and oversee care staff documentation. 
  • Assist in developing and maintaining a schedule for the caregiver staff that sufficiently meets the needs of residents and ensures proper staffing and adheres to Annual Budget.
  • Review daily all communication tools used in providing resident care. 
  • Communicate residents’ needs with caregiver staff at change of shift and with the Executive Director as needed. Review changes in residents’ condition with the Administrator, the resident’s representative, and resident’s medical providers. 
  • Lead by example, encourage teamwork, promote the Cumberland Hills philosophy, and provide ongoing coaching to employees. Ongoing presence out in the community and on the floor is required throughout the day in addition to administrative duties. 

Requirements

Required Skills/Abilities: 

  • Current State LPN or RN license. Maintain CPR certification.
  • A minimum of one year of experience working with residents in a long-term care setting. 
  • Ability to respond during off-duty hours to questions and emergencies as scheduled. 
  • Ability to interact and build relationships with older adults. Desire to work with older adults. 
  • Supervisory experience in healthcare highly preferred. 
  • Must be able to react in an emergency situation. 

Education and Experience:

  • Current license to practice nursing in the state.
  • Minimum of six month of recent experience as a professional nurse.

Physical Requirements: 

  • Stand/walk up to eight hours a day. Sit up to two hours a day 
  • Frequently support up to 75 pounds. Occasionally lift/carry up to 50 pounds. 
  • Frequently kneel, bend, and reach. 
  • Secure proper assistance for transferring of residents as needed
