Position Overview

We are seeking a dedicated and experienced Wellness Director to lead our health and wellness initiatives. This role is responsible for developing wellness programs that support physical, emotional, and social well-being.

Key Responsibilities

• Develop and manage individualized care plans for residents


• Ensure compliance with state and federal healthcare regulations


• Monitor resident health and coordinate medical interventions


• Design and implement wellness programs and activities


• Serve as a liaison between residents, families, and healthcare providers


• Maintain accurate health records and documentation

Qualifications

• Licensed Practical Nurse (LPN)


• 3–5 years of experience in senior care or assisted living


• Strong leadership and team management skills


• Knowledge of geriatric care and wellness principles


• Excellent communication and organizational abilities


• Familiarity with electronic health records (EHR) systems

Preferred Skills

• Certification in gerontology or wellness programming


• Experience with dementia and memory care


• CPR and First Aid certification

Working Conditions

• May include evening or weekend hours


• On-call responsibilities may be required


• Physical activity including walking, standing, and occasional lifting
