Supervisor: Housing & Wellness Program Manager 

Position Overview:

The Wellness Navigator position assists TGP members in accessing services and supports to meet their wellness, behavioral health, mental health, and physical health needs. The Wellness Navigator works within the Wellness program to facilitate connection to care options in the community and within the TGP continuum of care, assists in the coordination of services provided by partner agencies at TGP, and provides direct services to members.

Core Accountabilities:

• Support members in identifying their wellness needs and provide navigation services to overcome barriers to and facilitate connection to appropriate wellness services and programs  
• Is knowledgeable about other TGP programs and services and makes appropriate internal referrals 
• Keep up to date on resources to ensure quality referrals are being made to outside providers 
• Provide members with information about specialized providers, groups, and services to support their wellness and recovery needs 
• Help members navigate and obtain services and resources which includes but not limited to arranging appointments, completing applications, transportation, and coordination with partner agencies 
• Develop relationships with members that are marked with dignity to support wellness and recovery goals through peer support and direct member benefits 
• Provides crisis intervention and emotional support and encouragement to TGP members 
• Collaborates and communicates Wellness Program updates with TGP programs and external partners through meetings, e-mail or other forms of communication 
• Ensures completion of all program effort documentation and data entry as required, in a timely manner 
• Demonstrates respect, cultural humility and knowledge of trauma informed care in interactions with TGP members, staff and collaborative partners 
• Other duties as assigned by the supervisor

Requirements

• Motivated and self-driven 
• Ability to work autonomously or within a team 
• Organized, efficient, reliable, detail oriented 
• Excellent verbal and written communication skills; able to tailor communication to each audience 
• Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Teams, etc) 
• Knowledge of homelessness, poverty, women and trans issues, substance misuse a plus
