Position Summary

The Women's Imaging Patient Navigator will support the Nurse Navigator program, High Risk Screening Program and the Screening Mammography program. This position will provide assistance with scheduling biopsies, coordinating services, facilitate and confirm patient appointments, including those made with labs, diagnostic areas, and specialty physicians, manage referral and patient inbound and outbound messages, and collate pertinent statistics and documents needed for the navigation program. With the general guidance of the Nurse Navigator performs with initiative, ingenuity and independent judgement in coordinating administrative aspect of breast health navigating.duties include, but are not limited to, the following essential job functions:

Responsibilities:

• Work with health care professionals and support staff, including those from other disciplines, to provide patient-focused care.


• Continuous monitoring and communication of results, within specified timeframe, to patients as required for screening mammography program.


• Collate all needed documentation for referring providers, specialist providers, and further imaging procedures ie: pathology reports, imaging reports, medication lists


• Facilitate and coordinate appointments, within established service standards, for patients requiring biopsy procedures.


• Audit short term follow up exams (ACR 3) to ensure compliance


• Performs other related coordinator and navigator duties as assigned.

Qualifications:

• High school diploma or GED required


• Prior experience working in a hospital, ambulatory, or medical office setting preferred


• Strong communication and organizational skills


• Ability to multitask and work collaboratively in a team environment


• Basic clinical and computer skills preferred

Physical Demands

Light carrying under 15 pounds. Pushing, pulling, kneeling and bending required. Frequent reaching, some above shoulders, handling and fingering of instruments and equipment. Talking and hearing for communicating with other personnel and patients. Hearing to distinguish differences and variations of vital signs of patients. Near-visual acuity required to work with a charts and records and to observe patients to detect changes in patient conditions, to read gauges and dials on equipment. Color vision for perceiving changes in patient's skin color, color of medicines and solutions.
