Women's Health Coordinator

JOB SUMMARY

Works in collaboration with members of the health care team to reduce potential morbidity and mortality through cancer screenings by standardizing the timing and process of notification of abnormal pap and breast imaging results.   

DUTIES AND RESPONSIBILITIES:

• 
  Informs patient of abnormal test results.


• 
  Assists patients in referral process to a specialist for appropriate follow-up.


• 
  Makes contact with patient a week prior to additional imaging to remind the patient of that appointment.


• 
  Assists patient in reducing barriers that prevent patient from keeping scheduled appointments.


• 
  Refers patient to a Community Health Worker if patient requires additional assistance (i.e. transportation, insurance, food, etc.).


• 
  Begins application process for KWCSP if patient is eligible.


• 
  Retrieves copies of results from consults, diagnostic procedures, and pathology reports and inputs into the patients EHR.


• 
  Assist in training BSHC employees on KWCSP and processes.


• 
  Adheres to the terms and conditions set forth in BSHC’s corporate compliance program.


• 
  Ensures confidential information in accordance with BSHC’s policy and current HIPAA regulations.


• 
  Performs other duties as assigned.

MINIMUM QUALIFICATIONS / REQUIREMENTS:

• 
  Current licensure in Kentucky as a Registered Nurse or Licensed Practical Nurse.


• 
  Demonstrates a high level of written and verbal communication skills.


• 
  Ability to work in a friendly and courteous manner with staff and patients.
