WOUND CARE COORDINATOR (RN) – Full Time – Generations Oakton Pavillion, Des Plaines, IL

Generations at Oakton Pavillion in Des Plaines, IL, has an Immediate Opening for Full Time Wound Care Coordinator, RN.

Salary of up to $95k, Depending on Qualifications, Plus Full Benefits, including 401k w/Company Match, HSA, Teledoc, Choice of Insurance Plans, and much more!

The Schedule will be Monday-Friday, On-Call Clinical and Manager On Duty Rotations required; must work 3 holidays per year (1 Major and 2 Minor holidays).

*Wound Care Certification Preferred, but Not Required. We Will Sponsor Certification for the Ideal Candidate!*

Responsibilities of the Wound Care Coordinator (RN):

• Plan, implement and set priorities for wound care and prevention according to residents needs
• Medication application to existing wounds
• Educate patients and their families to care for their own wounds
• Care for patient's surgical incisions, any pressure sores or any wounds caused from disease or injury
• Skin evaluation for pressure sore prevention
• Stocking the treatment carts
• Complete MDS, careplan
• Work with wound physician on weekly rounds

Requirements

of the Wound Care Coordinator (RN):

• Current RN license from the State of Illinois
• Nursing experience at a SNF
• Wound Care experience required
• MDS and Care Plan experience a big plus
• Wound Care certification (WCC) or VOHRA preferred
• Personable and professional with residents, families, and staff
• Maintain confidentiality of all residents’ information in compliance with HIPAA standards
• Must have compassion, understanding and tolerance for older adults
