(YHDP)
Coordinated Entry Specialist
provides
direct program s
ervices
as a front

door for youth experiencing housing instability.
This program
provides traum
a

informed community-based
services
through housing assessments in order to identify available housing resources for youth
up to their 24
th
birthday.
Coordinated Entry Specialists will
assist clients in accessing st
able and safe
long-term
housing,
locating,
and identifying resources for youth, and coordinating supportive services.
Responsibilities
:
Primary responsibilities
include but are not limited to
:
Administrative Leadership
and Management
Follow all program policies and procedures as required to provide solution-focused housing interventions to expedite housing placement for clients.
Practice a Housing First philosophy with clients.
Implement a thriving
and
low
barrier
access point for youth seeking housing.
Complete quality
assessments to youth; in-person as needed.
Input data using HMIS and HUD qualifying standards.
Complete other documentation as required, including month-end reports.
Responsible for creating and maintaining community partnerships to assist in the service delivery identified as needed for clients’ success.
Responsible for making community connections to increase program referrals as assigned (distribute program brochures, schedule informational meetings, speak at community presentations, booth exhibits, etc.).
Participate in ongoing meetings/professional memberships and trainings as required by the Senior Director of Youth Housing.
Participate as an active team member with colleagues of all demographics in order to effectively and efficiently serve clients.
Maintain strong, open communication with direct supervisor regarding services delivered, concerns, or questions.
Qualifications
& Preferred Skills
Bachelor’s Degree (
preferred
) in social work, psychology, public administration, or a related human services field and three (
1
+) years of experience working in non-profit social service agencies with three (
1
+) years of management experience.
Previous experience working with at-risk
teen population
required.
Lived experience with housing instability preferred.
Demonstrated resourcefulness in setting priorities, proposing new ways of creating efficiencies, and guiding investment in people and systems.
Solid commitment to the principles of support and advocacy for
youth
.
The ability to collaborate and advocate for positive outcomes, using excellent interpersonal skills in multi-disciplinary, diverse, and dynamic professional teams.
Strong verbal communication skills and demonstrated ability to write clearly and persuasively.
Must have ability to quickly establish rapport and build trust.
Flexible and a self-starter; able to multi-task while also being highly detail-oriented while meeting all deadlines.
Developed organizational and assessment skills.
Work as a necessary part of a team effort with colleagues of all demographics.
Model ethical and professional behavior at all times.
Valid Missouri Driver’s License and car insurance
.
Travel required, with or without accommodation, to attend and/or present in-service trainings or presentations, Balance of State required meetings, and direct care to clients as needed.
Ability to be flexible in work hours to meet client needs.
