Job Description - YHDP Rapid Resolution Specialist

Position Summary


The Rapid Resolution Specialist plays a critical role in preventing and swiftly resolving homelessness among youth and young adults (YYAs) by identifying and facilitating Targeted Rapid Resolution (TRR) solutions. This position conducts assessments, evaluates immediate housing options, engages in problem-solving discussions, and leverages resources to divert, prevent, or rapidly exit YYAs from the homelessness system.


Key ResponsibilitiesAssessment & Eligibility for Targeted Rapid Resolution (TRR)



  • Conduct comprehensive, strengths-based assessments to determine if a YYA’s situation is a good fit for TRR interventions.

  • Identify natural supports, alternative housing options, and self-resolution strategies to prevent system entry when appropriate.

  • Utilize a trauma-informed and youth-centered approach to explore safe and sustainable solutions.

  • Ensure that TRR approaches align with Housing First, Positive Youth Development (PYD), and Harm Reduction principles.


Problem-Solving & Immediate Housing Solutions



  • Engage in solution-focused conversations with YYAs, family members, and other identified supports to resolve the housing crisis outside of formal shelter or transitional housing.

  • Facilitate conflict resolution, mediation, and family reunification when safe and appropriate.

  • Assist YYAs in navigating available short-term assistance, including rental support, move-in assistance, or emergency placements.

  • Coordinate with Coordinated Entry (CE) partners, outreach teams, and emergency responders to streamline responses and prevent system entry.


Resource Coordination & Service Navigation



  • Connect YYAs to flexible financial resources, including transportation assistance, reunification funds, or temporary housing support.

  • Maintain up-to-date knowledge of local housing resources, crisis response services, and diversion funds to inform decisions.

  • Work closely with housing providers, case managers, and youth-serving agencies to ensure seamless service transitions.

  • Support YYAs in accessing supportive services, such as employment, education, mental health, and substance use treatment programs.


Data Management & Reporting



  • Document all interactions, assessments, and housing resolutions in the Coordinated Entry System (CES) and case management system.

  • Track and report program outcomes, resolution rates, and barriers to inform service improvements.

  • Contribute to continuous quality improvement efforts by identifying trends and best practices in rapid resolution strategies.


Training & Community Engagement



  • Provide training and technical assistance to outreach teams, youth-serving agencies, and crisis response providers on TRR strategies.

  • Advocate for system-level improvements to enhance the effectiveness of rapid resolution strategies.

  • Engage in multi-disciplinary collaboration to expand the reach and success of rapid resolution efforts.


QualificationsRequired:



  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (or equivalent experience).

  • 2+ years of experience working with youth/young adults experiencing or at risk of homelessness.

  • Strong knowledge of homelessness diversion, crisis intervention, and family mediation.

  • Experience utilizing trauma-informed, youth-centered, and harm-reduction approaches.

  • Ability to navigate conflict resolution, mediation, and problem-solving with empathy and patience.

  • Knowledge of Coordinated Entry Systems (CES), Housing First principles, and diversion strategies.

  • Proficiency in case documentation, data tracking, and reporting.


Preferred:



  • Lived experience of homelessness or system involvement (with professional experience).

  • Familiarity with Rapid Rehousing, Transitional Housing, and Permanent Supportive Housing programs.

  • Experience with Apricot, HMIS, or similar case management systems.

Work Environment & Expectations



  • Hybrid work environment with in-person, field-based, and remote responsibilities.

  • Some evening and weekend hours may be required based on youth needs.

  • Travel within the community to meet with youth, partners, and housing providers.
