Youth Emergency Shelter Case Manager

Schedule: Full time, benefits-eligible! Monday through Friday 8am-5pm but will require flexibility based on client and program needs.

Demonstrably bilingual (English-Spanish) applicants are eligible to receive an additional $1 / hr.

Program Mission

To recognize and meet the distinctive needs of youth ages 18-24 who are experiencing homelessness by providing stable housing, mental health care, case management, life skills training, and connection to community services and resources with the ultimate goal of assisting all clients in obtaining and sustaining safe and stable housing.

Position Summary:

The Emergency Shelter Housing Case Manager provides direct support and housing-focused case management services to individuals experiencing homelessness. This role focuses on stabilization, connection to community resources, and connection to safe, sustainable housing resources. The Case Manager works collaboratively with clients, shelter staff, and external partners to ensure housing goals are met and barriers are addressed effectively.

Essential Duties

• Maintain a caseload of up to 20 shelter clients.
• Assist clients in developing goals/service plan that will help them resolve their homelessness, identify and address barriers to housing.
• Meet/check-in with case managed clients at least once a week (All case notes entered in a timely manner and with consistent documentation methods).
• Hold daily office hours on the shelter floor (minimum two hours per shift) to ensure client accessibility and engagement.
• Support clients through every step of the housing process, including completing applications, requesting application fees, and submitting materials to property managers. Assist clients in scheduling and attending appointments related to housing searches and placements.
• Participate in clinical case reviews to ensure appropriate service planning and referrals to internal or external providers.
• Deliver trauma-informed case management, including crisis intervention, risk assessments (e.g., SPDAT, Lethality Assessment), goal setting, resource navigation, and care coordination.
• Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and coordinate services with diversion as needed.
• Attend weekly staff/clinical staffing meetings and other meetings as assigned.
• Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices. Also provides immediate and professional response to any security /safety emergency or disturbance within or immediately outside facility following appropriate policies and procedures.
• Keep records, case management notes, and statistical data as required and participate in research and/or special projects as needed.
• Transport clients to and from appointments, which may include one-on-one sessions.
• Attend community outreach events as needed.
• Leading groups or meetings with clients confidently and effectively.

Secondary Duties

• Maintain ethical, professional conduct by upholding VOA Utah’s Code of Ethics, working respectfully with diverse populations, and maintaining appropriate boundaries with clients, volunteers, and colleagues.
• Act and make decisions within the scope of position responsibilities.
• Accurately and timely collect all client information for required databases: UWITS, HMIS, etc...
• Proactively expand and integrate knowledge of community resources.
• Protect the safety of all information handled.
• Other duties as assigned by supervisor.

Qualifications

High school diploma or equivalent required and 2 years of social services or homelessness-related experience or any combination of education, experience, and training which provides the following knowledge, skills and abilities:

• Familiarity with Trauma Informed Care, Motivational Interviewing and Housing First philosophies and willingness to incorporate them in your daily work.
• A degree in the field of social services and two years’ experience working in the social services field preferred.
• Must be able to qualify and pass Utah Department of Human Services Case Management Certification within 60 days of being hired.
• Must obtain CPR certification once hired (45 days).
• Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public.
• Ability to utilize Trauma Informed practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision for major situations.
• Willingness to accept supervision and direction.
• Driving is required: must be at least 21 years of age, possess a current driver’s license, have a good driving record and be insurable on the agency liability policy.

Physical Demands

• Ability to lift and carry 50 lbs.
• Ability to move around the facility, interacting with clients for extended periods.
• Work is generally performed in an office environment. May entail using a computer for extended periods of time.
