Job Description - Youth Interventions Pathway Case Manager

Description

Annual Salary $44,000

SGA provides a wide range of social services in some of Chicago's most challenged and consistently underserved communities and creates long-term and community-wide change.

MAJOR DUTIES AND KEY RESPONSIBILITIES

  • Administering risk assessment tool, conducting intakes with youth and family, case planning, referrals, follow-up, and case closure.
  • Contacting youth through phone calls, home visit, schools visits, or meeting in a neutral location encourage engagement in services.
  • Utilize the YASI and work with youth to identify their strengths, needs and a pathway to connecting them to support and services.
  • Empower youth in decision-making and work from a strengths-based perspective that emphasizes agency to develop youth plans that will be completed over a 90-day period.
  • Engage families, as appropriate, and purse family service connections (e.g., to healthcare, housing supports, etc.) that support youth well-being.
  • Enter data into Cityspan to track and monitor youth outcomes
  • Communicate with referral partner within 48 hours/2 business days of services
  • Inform referral partners of any changes to youth’s circumstances that impact service delivery.
  • Maintain between a 30 and 35 caseload.
  • Conduct outreach to the community (individually and as a team member) to build strong relationships with youth, residents, businesses, and community groups.
  • Form relationships with high-risk youth in the community.
  • Report to the YIP Program Coordinator weekly
  • Execute program activities to the highest of qualities.
  • Work with team members to coordinate an inclusive and immediate strategic response when violent activity occurs.
  • Assist in mediating situations and preventing retaliation between individuals and groups.
  • Actively participate in all mandatory program trainings and weekly staff meetings, agency convening, as assigned.
  • Provide participants with support and referrals to programs and opportunities in the community (e.g., job programs, GED, drug treatment, and mentoring.)
  • Other duties as assigned.

Requirements

  

Education:

  • Associate Degree accepted or Bachelor’s Degree in Social Service or related field is (Preferred).

Experience:

  • Extensive experience working with high-risk youth and gang members.
  • Excellent verbal and written communication skills
  • Experience and/or training in crisis intervention
  • Residence within the target area preferred
  • Ability to work a flexible schedule that includes late evenings, weekends, and holidays; set schedule of 2pm-10pm.
  • Committed to working toward positive community change.
  • Must be able to pass a background check, inclusive of CPS.

Other:

  • Sensitivity to cultural diversity is required.
  • Ability to communicate and cooperate with diverse families, various professionals and community groups.
  • Comprehensive knowledge of neighborhood revitalization efforts.
  • Experience and/or a clear understanding of the community(s) being served.
  • Demonstrated track record of strong community engagement skills.
  • Ability to perform and complete multiple tasks at the same time.
  • Excellent verbal and written communication skills.
  • Excellent analytical and organizational skills.
  • Unquestioned personal and professional integrity and dedicated work ethic.
  • Innovative and analytic problem-solving abilities.

KEY COMPETENCIES

  • Proficient in Microsoft Office tools and inputting services in an online database.

PHYSICAL REQUIREMENTS

  • Must be able to remain stationary 50% of the time.
  • Must be able to frequently move inside the office to access file cabinets, office machinery, etc.
  • Occasionally ascends/descends stairs.
  • At times may be required to lift up to 25 lbs. for various classrooms and event’s needs.
  • Frequently moves in outdoor weather conditions.
