Job Description - Youth Program Coordinator and Therapist for IIBHT

 


 Center for Human Development is seeking a Youth Program Coordinator and Therapist for In Home Behavioral Health Treatment which is responsible for planning, organizing, and directing the In-Home Behavioral Health Treatment (IIBHT) program. Responsible for promoting community awareness of program and facilitating client services and coordination of care. Provides counseling and individual, family therapy and psychoeducation


 


CHD WILL PAY UP TO $3500 IN RELOCATION ASSISTANCE AND UP TO $2000 TOWARDS FIRST MONTH RENT!

The Requirements:



  • Master's degree in a social sciences field. *

  • State licensure or be willing to pursue licensure preferred.

  • Two years of relevant mental health experience.

  • Must meet requirements of a Qualified Mental Health Professional.

  • Certified as an Alcohol and Drug Counselor or willingness to pursue.

  • Knowledge of practices, principles and philosophy of mental health treatment including motivational interviewing, assessment, treatment plans, and crisis interventions.

  • Knowledge of computer basics.

  • Communicate effectively verbally and in writing.

  • Proficient in coordinating services with care providers from other agencies.

  • Ability to provide adequate written documentation of services rendered in order to satisfy federal, state, and ACT requirements.

  • Ability to develop documents, spreadsheets, and presentations that illustrate and track how ACT services are being implemented and improved.

  • Valid driver’s license, and must pass criminal history check and drug screening.

  • If master’s degree has not been obtained, to be considered for this position you must be attending school and have at least 30 credits based on quarter completed, or 20 credits based on semester completed


Essential Job Functions:



  • Engage families in intake and orientation process.  Services should be provided in the home, or in the environment the family chooses.

  • Collaborate with local community members, schools, physical health and alcohol and drug treatment providers and other organizations necessary to promote and establish youth care program activities.

  • Facilitate care coordination meetings and develop behavior support and safety plan and implement service coordination plan.  

  • Promote community awareness of youth care issues specific to child and adolescent risk factors. 

  • Collect data and report on the state website. 

  • Be available for crisis support 24/7 for assigned programs (via phone and to be shared among team members).

  • Provides individual, family and group counseling and psychotherapy.

  • Other duties as assigned.
