Job Description - Youth Wellness Navigator


Summary:
The Youth Wellness Navigator (YWN) provides suicide-related, trauma-sensitive, and person-centered support to youth and their families. This position will work with youth who are at high risk for suicide, providing individualized intervention and system navigation services. This position is additionally tasked with delivering public-focused education on suicide prevention. The YWN is community-based, with an office in a local library utilizing community partnerships to provide client services and public education

Location: Sheboygan

Work Week: M-F; 40 hours per week; including some after school hours 

Pay Rate $19- $26 per hour

Essential Functions
  1. Facilitate one-to-one sessions with identified clients.
  2. Implement evidence-informed programming with youth at high risk for suicide.
  3. Develop sustainable models to support transition, follow-up, and resource navigation for youth at risk for suicide, including prevention and postvention resources.
  4. Work with program participants using a cognitive-behavioral approach to aid clients in learning the tools to overcome life’s challenges.
  5. Work with clients and collateral sources to develop goals and intervention plans to assist with client goal improvement and ensure bio-psycho-social needs are met.
  6. Establish a case management protocol for clients from direct interaction or referrals.
  7. Provide psychoeducation to parents and youth regarding mental health wellness and suicide prevention.
  8. Utilize a trauma-sensitive, person-centered, and zero-suicide approach when working with individuals and their support.
  9. Develop and maintain positive relationships with community partners and stakeholders that provide referrals and services to youth.
  10. Ensure thorough documentation and accurate, timely data entry into the agency’s electronic case management system.
  11. Maintain training, education, and best practice requirements, per WI Chapter 34 and any credentials held by the individual.

Qualifications

  1. Bachelor’s Degree in mental health or related field.
  2. Knowledge of behavioral health, substance use, suicide, and co-occurring diagnoses.
  3. Strong written and verbal communication skills.
  4. Strong critical thinking and problem-solving skills.
  5. Experience or training in crisis intervention preferred.
  6. Experience or training in working with youth and families.
  7. The ability to demonstrate cultural humility.
  8. Strong understanding and implementation of self-care practices.
  9. Knowledge of federal, state, and local laws, rules & regulations affecting crisis intervention.
  10. Ability to collaborate and work as a part of an interdisciplinary team.
  11. Personal qualities of integrity, credibility, and a commitment to and passion for Elevate’s mission.
  12. Pre-employment criminal background check, employment background check, driving record, and drug screening required.
