YOUR DAY-TO-DAY

Here's what you need to become our Care Management Specialist:

• Master's degree in a health-related field of counseling, psychology, social work, or other behavioral sciences
• 2+ years of Behavioral health experience

This full-time behavioral health position works Tuesday-Saturday schedule from 8 am until 5 pm .

As a Care Management Specialist, your days are a blend of professionalism and compassion. You provide crisis assessment and counseling, ensuring efficient data entry. You greet and assist clients, collaborate with law enforcement, and connect individuals to resources. Your role extends to care management, focusing on person-centered plans, care coordination, social support, and patient/family support. You also offer transitional care and health promotion, both in the office and the community, making a positive impact on individuals and families every day.
