Job Description - Clinical Pharmacist

The Staff Pad has partnered with a growing healthcare organization in Portland, OR looking to add a Clinical Pharmacist to their team. As a Clinical Pharmacist, you’ll play a vital role in providing direct patient-centered care and integrated pharmacy services. From leading therapeutic programs to delivering clinical consultations, youll have the opportunity to shape the future of pharmacy practice in a dynamic and collaborative environment.

 

Essential Duties:

Lead patient service lines and therapeutic programs to ensure optimal medication therapy.

Collaborate with interdisciplinary teams to provide comprehensive pharmaceutical care aligned with evidence-based best practices.

Develop policies and procedures to enhance drug utilization and patient safety.

Deliver direct patient care and clinical practice, including decentralized and service-based programs.

Identify and implement interventions to improve quality and safety, making evidence-based medication recommendations.

Provide discharge medication review, reconciliation, and counseling to promote medication adherence.

Maintain proficiency in electronic health record systems and medication ordering systems.

Communicate effectively with healthcare providers and caregivers to ensure seamless patient care.

Participate in pharmacy operations and medication dispensing, ensuring accurate and timely medication therapy.

Facilitate specialty medication procurement, ordering, and dispensing procedures.

Identify and implement improvements in the medication-use system to advance patient safety and therapeutic outcomes.

Qualifications:

Doctor of Pharmacy degree from an ACPE accredited College of Pharmacy.

Completion of ASHP accredited PGY-1 Pharmacy Residency.

Two years of clinical pharmacist experience required if residency is not completed.

Strong interpersonal, organizational, and decision-making skills.

Excellent verbal and written communication skills.

Ability to work independently and as part of a team, with flexibility in approach to time management.
