As our Social Worker at Lasting Hope , you’ll perform social work assessments and interventions for inpatients, outpatients, and emergency department patients. Working collaboratively with all healthcare team members, you will develop and implement treatment plans that support patient-centered care for individual patients and the medical community.

A 64-bed adult psychiatric facility Lasting Hope Recovery Center is a place of healing. All of the rooms are private. At Lasting Hope Recovery Center patients find comfort and a team of caring professionals working for them. Often mental illness and substance abuse occur at the same time. CHI Health Lasting Hope Recovery Center offers an entire range of treatment and prevention services to meet the needs of patients experiencing both challenges. Treatment is built on personal strengths and support systems that treat individuals experiencing these illnesses.

 

 

• Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations
• Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families. 
• Serve on hospital committees and develop and present in-services to Hospital staff.
