Job Description - Clinical Case Manager

Description

GENERAL FUNCTIONS:

Clinical Case managers serve as a member of the multi-disciplinary team responsible for providing recovery-oriented services to clients who are dually diagnosed with mental illness and developmental disability. Some clients present with co-occurring physical and/or substance abuse issues.

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Essential Responsibilities, include but are not limited to:

  • Conduct Mental Health Assessments and Semi-annual treatment planning for assigned clients using the State approved IM/CANS tool ensuring the treatment plans incorporate objectives that reflect the identified needs of the individuals served.
  • Provide counseling/therapy to clients based on assessed needs.
  • Oversee the submission and retention of SSI and LINK benefit status/approvals.
  • Provide clients with referral, linkage, and follow-up services to needed resources, such as housing, entitlements, educational, vocational and guardianship services.
  • Provide information to outside agencies as requested to coordinate and assure the appropriateness of services.
  • Provide individual and group skill building activities with a substantial percentage in the client’s natural environment: home, school and community.
  • In conjunction with the client’s treatment team, keep the family, guardian and other Envision Unlimited staff informed of the client’s progress.

The noise level in the work environment is usually low to moderate. The work environment may include any or all of the following: working alone; working with others, verbal contact with others and face-to-face contact.

Requirements

QUALIFICATIONS:

Bachelor’s degree in social work, counseling, psychology or related field. 1-3 years of experience in working within the social service arena, including internships. Must be able to work with diverse populations with multi-faceted needs. Experience with mental health and I/DD populations and knowledge of Medicaid standards. Bilingual: Spanish & English preferred.

Must be at least 21 years of age, possess a valid driver's license or state ID, be approved to drive agency vehicles through agency insurance, pass a criminal background check. Must have excellent written and oral communication skills.
