• Patient Volume/Census: 8–12 patients per day
• Admissions: Yes – responsible for admissions and discharges

• Case Mix: Geriatric and long-term care patients
FACILITY INFORMATION
• EMR System: Point Click Care and Epic
• Specialty Backup Available: MDs and APPs
• Support Staff Available: APPs, Nursing staff
• Reason for Coverage: Vacancy / Supplemental coverage

JOB REQUIREMENTS
• Licenses: Active Vermont medical license
• Board: BC/BE in Family Medicine, Internal Medicine, or Geriatrics
• DEA: Active DEA Certification
• Certifications: BLS
• Experience: Preference for geriatric experience (not required)
• Other Qualifications: Familiarity with EMR systems (Point Click Care & Epic)
DUTIES & RESPONSIBILITIES
• Provide quality patient care in a nursing home setting
• Manage admissions, discharges, and acute evaluations
• Conduct 30-day evaluations for residents
• Collaborate with APPs, MDs, and nursing staff
• Participate in on-call duties as needed
