Job Description - Family/ Internal Medicine Physician- (Geriatrics)

JOB OVERVIEW

• Job Title: Physician – Family Medicine / Internal Medicine / Geriatrics

• Job Type: Locum Tenens

• Location: Berlin, VT

• Service Setting: Nursing Home

• Coverage Type: Clinical + Call

• Coverage Period: 10/08/2025 – 12/31/2025 (with potential for extension)

• Clinical Shift Schedule: Mon – Fri, 8a – 5p (Option for Mon–Thu or Tue–Fri schedule)

• On-Call Shift Schedule: Flexible

• 10/08/2025 – 12/31/2025

PATIENT INFORMATION

• Patient Demographics: Geriatric patients

• Patient Volume/Census: 8–12 patients per day

• Admissions: Yes – responsible for admissions and discharges

• Rounding: Included

• Phone Consults: Yes

• Case Mix: Geriatric and long-term care patients

FACILITY INFORMATION

• EMR System: Point Click Care and Epic

• Specialty Backup Available: MDs and APPs

• Support Staff Available: APPs, Nursing staff

• Reason for Coverage: Vacancy / Supplemental coverage

COMPENSATION & BENEFITS

• Rate: TBD

• Travel, Lodging, and Malpractice Insurance: Covered

REQUIRED PROCEDURES

• Admissions

• Discharges

• Acute evaluations

• 30-day evaluations

JOB REQUIREMENTS

• Licenses: Active Vermont medical license

• Board: BC/BE in Family Medicine, Internal Medicine, or Geriatrics

• DEA: Active DEA Certification

• Certifications: BLS

• Experience: Preference for geriatric experience (not required)

• Other Qualifications: Familiarity with EMR systems (Point Click Care & Epic)

DUTIES & RESPONSIBILITIES

• Provide quality patient care in a nursing home setting

• Manage admissions, discharges, and acute evaluations

• Conduct 30-day evaluations for residents

• Collaborate with APPs, MDs, and nursing staff

• Participate in on-call duties as needed
