Family or Internal Medicine Physician – Diabetes & Lifestyle Medicine Specialist, San Antonio, Texas
Employment Type: Full-Time (36 hours/week; flexible 4- or 5-day schedule)
Position Overview:
Join a mission-driven, patient-centered team dedicated to transforming health outcomes through holistic care. Health by Design is seeking a board-certified or board-eligible Family or Internal Medicine Physician with a passion for diabetes management and lifestyle medicine. This newly created role focuses on caring for a unique and rewarding population—retired First Responders—within a value-based care model that emphasizes prevention, wellness, and sustainable lifestyle change.
What You’ll Do:
Deliver comprehensive care to patients with diabetes and related chronic conditions. Create individualized care plans rooted in nutrition, exercise, stress reduction, and behavior changes. Collaborate with a multidisciplinary team of dietitians, diabetes educators, pharmacists, and care coordinators. Utilize evidence-based practices and advanced diabetes treatments to drive measurable outcomes. Empower patients through education and coaching around self-management and lifestyle interventions. Document and monitor care quality using efficient, up-to-date EHR practices. Stay current with innovations in lifestyle medicine, diabetes care, and value-based health models.
Schedule & Locations:
36-hour work week with administrative time built in (no call, no after-hours charting)Flexible 4- or 5-day scheduleRotate between four clinics: Westover Hills, Fair Oaks, Roosevelt (South), and Schertz.
Qualifications:
MD or DO, BC/BE in Family Medicine or Internal MedicineTexas medical license (or eligibility).
Strong foundation in diabetes care and interest in lifestyle/wellness-based medicine
Excellent communication and interpersonal skills
Value-based care experience and EHR proficiency preferred
