Job Description - Medical Case Management Intern (Fall 2025)


GENERAL:
Under the administrative/clinical supervision of the Medical Case Management (MCM) Supervisor/Field Instructor, the Medical Case Management Intern provides services to clients, their families and significant others.  Alongside our Medical Case Manager you will not only assist in the provision of individual interventions for clients, but you will also work closely with a client’s family, extended family, medical team, DCFS caseworkers and other professionals involved in providing services and support to the child and family.

ESSENTIAL DUTIES:
  1. Facilitate initial meetings with families to educate about the MCM program.
  2. Complete initial paperwork and development of goals.
  3. Complete ongoing clinical documentation including progress notes, ISP, closing paperwork.
  4. Communicate and coordinate services with DCFS workers.
  5. Meet with families on regular basis in their home.
  6. Meet with families monthly at a medical appointment.
  7. Communicate and coordinate services with hospital social workers or other medical staff as needed.
  8. Participate in weekly supervision meetings.
  9. Attend monthly reviews with DCFS and MCM team.
  10. Engage in other services necessary to the enrichment of the internship experience.
  11. All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  12. Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
  13. Respect the privacy of clients and hold in confidence all information obtained during the client’s treatment.  All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  14. Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  15. Other duties as assigned by management.
QUALIFICATIONS:
  1. Education: Minimum Bachelor’s Degree required. Must be enrolled in the second year of a Master’s program in Social Work, Counseling, or Marriage & Family Therapy.
  2. Licensure: SWT, CT or LSW required.
  3. Skills/Competencies:
  • Strong clinical skills including training and/or classroom experience expertise in therapy, crisis intervention, education, behavioral interventions, and substance abuse therapy.
  • Ability to perform job responsibilities with a high degree of initiative and independent judgment.
  • Core Expertise: Possesses skill, knowledge and abilities to perform the essential duties of their role; keeps knowledge up to date.
  • Cultural Competency: Demonstrates awareness, sensitivity and skills in working professionally with diverse individuals, groups and communities who represent various cultural and personal background and characteristics.
  • Interpersonal Communication: Communicates clearly using verbal, nonverbal, and written skills in a professional context; demonstrates clear understanding and use of professional language.
  • Professional & Ethical Conduct: Adheres to professional values such as honesty, personal responsibility, and accountability; applies ethical concepts within scope of work and adheres to Agency policies and procedures.
  • Collaboration & Teamwork: Functions effectively as a member of a professional team that includes employees, clients and family members.
  • Problem-Solving & Decision-Making: Recognizes problems and responds appropriately; gathers information
  1. Experience: Experience working with children, adolescents and their families
  2. Other: Must have and maintain a valid drivers license and driving record that meets the underwriting criteria of the Agency’s insurance company. Must have flexible schedule.
