SUMMARY Under general direction, is responsible for providing home assessments and recommendations based on accessibility needs and durable medical needs within the home. Duties and activities may vary based on assignments.

ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Physical Therapist – ADAPT Accessibility Consultant will include, but are not limited to the following: Travel within the five boroughs. Will support Family Support Services, Doorways to Independence program by visiting homes of applicants to assess person and home accessibility needs. • Will maintain communication with contractors as needed, and coordinate completion of projects with Program Director and Home Modification Committee. • Provide brief evaluation on individuals that are pertinent to the accessibility issues in areas of muscle tone and/or strength, range of motion, reflex patterns, gross motor capacity, posture and joint alignment, patterns of locomotion and/or ambulation, activities of daily living, including transfers and wheelchair management, need for adaptive equipment and recommendations for changes in present equipment and changes in home environment through accessibility modification. • Develop home modification evaluation based on assessment of the person’s ability and home accessibility needs. • Provide Home Assessment evaluation/report, photos of property and recommendations of various possible modification in the home. • Participate in in-service education /training that is relevant to techniques and objectives of program and in accordance to ADAPT policy. • Participates in interdisciplinary team meetings, consultation with parent/guardian and staff at team meetings, case conferences or through other informal contacts and annual reviews as required by the program. • Performs other related duties as requested.

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Ability to read, write, speak and understand English • Good interpersonal skills are required in order to interact effectively with employees, coworkers, consumers, their families, other programs, outside contacts, etc. • Due to sensitive nature of the position, must be able to maintain the strictest confidentiality. • Ability to work independently and handle multiple tasks. • Good organizational skills • Good oral and written communications skills • Analytical ability necessary to evaluate consumer’s condition and recommend appropriate accessibility modifications in the applicant’s home.

EDUCATION and/or EXPERIENCE • Master’s in Physical Therapy. • Five years of prior experience as a Physical Therapist preferred • Prefer experience across multiple settings and working with all age groups, assessing, evaluating, and designing individual treatment and plan of care for various medical diagnosis. • Prefer working knowledge of durable medical equipment and home modifications to allow patients to remain and function as independent as possible in their own homes. • Prefer Certificate in Home Modifications, specifically designed program for professionals who work directly in the field of supportive home environments. • Prefer some years of experience in the construction industry on general contractor projects, with experience in electrical projects for both residential and commercial settings.

LICENSE/CERTIFICATION Current NY State license in Physical Therapy

RATE -$54/hour
