Certified Occupational Therapist Assistant (COTA)

3 - 8hr shifts every week. Rotating weekends and holidays
COTA provides direct patient care for a d iverse community-based caseload with the observation of care provided directed by an Occupational Therapist. Provide s subject matter expertise and proficiency of clinical best practices and achieve standards for patient satisfaction and outcomes for care delivered.
Responsib ilities:
- Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.
- Consistently utilizes a patient-centered approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.
- Treats patients/clients to relieve pain, develop or restore function and maintain maximum performance using specified occupational therapy procedures.
- Directs and aids patients/clients in plan of treatment.
- Utilizes physical agents including heat, cold, water, electricity, ultrasound and massage.
- Observes and reports to the Occupational Therapist and organizational personnel the patient/client reaction to treatment and any changes in patient/client condition.
- Instructs patient/family/caregivers in care and use of prosthetic device, therapeutic aids, splinting, and wrapping techniques as needed.
- Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
- Delivers quality care consistent with Home Health Conditions of Participation, qualifying home care criteria, payor coverage criteria, and providing visits/treatments consistent with the plan of care.
- Conducts clinical review of records to include multi-disciplinary review, coordination of care, provided in conjunction with visits and documentation.
- Identifies risk for acute hospitalization and proactively prevents adverse events.
- Participates in case conferences, in-service programs and other meetings required to ensure coordinated and comprehensive care. Achieve certification in Integrated Care Management within 1 year of hire. Certification includes completing the Integrated Care Management formal training session, successfully completing assigned computer based-learning modules within 2 weeks of training, and a competency evaluation. Use the Integrated Care Management philosophy, skills, tools and documentation in clinical practice. Demonstrate competency in selected ICM skills annually.
- Graduate of State Board approved program for Occupational Therapy Assistants and valid license as an Occupational Therapy Assistant.
- Experience with Microsoft Office software applications.
- Minimum CNA 75 hour
- One year experience in a health care field.
- Strong interpersonal skills.
- Ability to work as a collaborative team member.
- Ability to understand and apply guidelines, policies and procedures.

- Knowledge of rehabilitation principles and practices including the ability to carry out the procedures in the home.
- ICM Certification
- Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
- Mandatory Reporter: Child & Dependent Adult Abuse.
- Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities.
- CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
