Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. Assertive Community Treatment (ACT) is an evidenced based practice model that provides an intensive and highly integrated approach to community mental health services. ACT support helps people with severe and persistent mental illness become independent and live in the community as they work toward their recovery goals. ACT support also helps with reducing homelessness and unnecessary hospital stays. 

The Adult ACT Medication Training and Support (MTS) Specialist provides outpatient community-based counseling support to educate; problem solve and develop strategies to incorporate prescribed medication into daily routine and lifestyle. Additionally, they will assist clients with improving mental and physical health symptoms and behaviors that otherwise interfere with daily functioning and wellness goals. This is a per diem position - hours vary based on need.

Major Functional Areas and Description of Duties

• Provide Medication Training and Support (MTS) services to educate, problem solve and develop strategies to incorporate prescribed medication into daily routine.
• Deliver counseling services in the home or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.
• Develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with      functioning and goal achievement.
• Provide Family Training and Counseling (FTC) services, as needed, to educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals.
• Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.
• Work collaboratively with other ACT Team members, natural supports, and external providers to ensure wrap-around services are in alignment with the ACT model.
• Adhere to and uphold the General Organization Index and Fidelity Scales specific to IMR and ACT to support the authenticity of program implementation.
• Serve on appropriate committees, both internal and external, as determined by supervisor.
• Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.
• Execute all required services and timely documentation in accordance with agency policies and federal/state standards.
• Maintain professional memberships, licenses, accreditation, certifications, etc.
• Represent CLM in a professional and appropriate manner in all settings.
• Perform all other duties as assigned by supervisor.

•   Bachelor’s degree preferred (nursing, psychology, social work, clinical mental health counseling) however an Associate’s degree (human services, LNA, CNA) along with a minimum of two years of direct service experience in a related field may qualify.
• Experience working with co-occurring substance use disorders preferred.
• Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.
• Proficient computer skills, specifically with all Microsoft office applications.
• Ability to document effectively and efficiently in course of clinical work.
• Ability to navigate an electronic health record.
• Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision.
• Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services.
• Effective communication skills, organizational skills, and ability to multi-task.
