Are you passionate about empowering adults with intellectual and developmental disabilities to live their best lives? Join our dedicated team and be the bridge between individuals, families, and the resources they need to thrive.

What You’ll Do

As an Adult Case Manager, you will:

Build trusting relationships with individuals and families

Conduct in person meet and greets with prospective participants and families

Advocate for client rights and access to services

Maintain accurate, timely documentation to meet compliance standards

Collaborate with providers, agencies, and internal teams to ensure quality care

What We’re Looking For

Available 20 hours per week

Bachelor’s degree in Social Work, Psychology, Human Services, or related field (or equivalent experience)

Prior work with I/DD populations preferred

Strong communication, organization, and problem-solving abilities

Compassionate, resourceful, and committed to person-centered care

Valid driver’s license, reliable transportation
