Job Description - Advanced Practice Clinical Coordinator

Description

The Advanced Practice Clinical Coordinator (APCC) is responsible for establishing, implementing, and monitoring clinical programs to ensure safe administration of therapies to patients. The APCC collaborates with internal and external stakeholders to provide initial and continuous education. 

Reports to:  VP of Clinical Services 

Job Type: Full-Time

Requirements

  • Master’s degree in nursing 
  • NP Certification 
  • Registered Professional Nurse with current State Licensure, CDS and DEA 
  • Prior Work Experience as a Professional Nurse  
  • Basic Life Support (BLS)  
  • Thorough knowledge of bleeding disorders, rare diseases, and specialty infusion therapies preferred 
  • Active Membership in a professional organization preferred
  • Active DEA Number
  • Specialties in Adult-Gerontology Acute Care, Pediatrics preferred

Job Responsibilities, included but not limited to:

  • Develop and ensure that clinical Care and operations comply with policies of PromptCare’s treatment of patients
  • Report on activities, progress, challenges, and opportunities with all aspects of current and new therapies
  • Provide clinical education and presentations regarding specialty therapies to patients, providers, and staff
  • Provide effective and safe teaching using patient centered care approach
  • Effectively communicate with the Clinical team, providers, patients and families to facilitate continuity of care and ensures timeliness of communications
  • Perform proactive risk assessments for new therapies and develop standards of practice and clinical protocols
  • Facilitate clinical communication with referral sources and payors
  • Work collaboratively with Sales/Marketing to promote Promptcare clinical services
  • Implement appropriate clinical decisions and demonstrate appropriate use of expert peers and/or management, as applicable
  • Oversee the Nurse Clinical review in Appian
  • Participate in the Promptcare Quality Improvement (QI) Program including QI project management and occurrence reporting and review
  • Provide oversight of the Patient Management Program
  • Assit in marketing or Promptcare products and services as required
  • Participate in sales, marketing, and education programs as directed
  • Assist in obtaining/completing billing information and documentation as required
  • Perform duties of nursing personnel when required
  • Ensure compliance with all Promptcare policies and procedure, state and federal regulatory requirements and accreditation standards
  • Maintain inter-departmental communication and collaboration to facilitate care coordination as appropriate
  • Operate within scope of practice as defined by federal and state laws and regulation, practitioner competencies and standards set forth by relevant professional associations
  • Other duties as assigned
