Job Description - Advocate Assisant

Description

Position Summary:

Will be responsible for assisting elderly client caseload in need of home care hoarding services. The Safe and Secure Advocate Assistant will have contact with elderly clients in various hoarding situations. Safe and Secure Advocate Assistant will act as a support for clients in areas of sorting, disposing, donating items stated in Hoarding Plan. This plan will be developed by Safe and Secure Advocate and implemented in part by Assistant. Safe and Secure Advocate Assistant will interact with various necessary community agencies.

Duties And Responsibilities:

  • Provide community outreach and provide resource information on Safe and Secure
  • Collaborate with community resources
  • Documentation of all outcomes
  • Provide advocate support to link clients to services.
  • Encourage an appropriate and effective support system for clients.
  • Maintain contact with client, service providers and client’s family members, as appropriate and requested
  • Follow Hoarding Care Plan.

General Duties and Responsibilities

  • Complete annual Court/Social Security/VA reports on a timely basis.
  • Complete all agency and contractual paperwork on a timely basis, keep clients records updated and current, and provide all necessary paperwork required to prove that services are being provided.
  • Follow contract and best practice guidelines for clients regarding home visitation and personal contact.
  • Advocate for client’s needs in the capacity assigned.  
  • Responsible for On-call phone as assigned by director.
  • Serve on agency committees as assigned.
  • Will attend CPR and First Aid trainings and will keep certifications current.
  • Will attend all mandated trainings required and will keep certifications current.
  • Other duties as assigned.

Requirements

Qualifications:

  • Ability to complete multiple duties and tasks correctly and on time without being prompted.
  • Ability to learn and use program specific computer applications.
  • Ability to work with team members and others to research, plan and implement initiatives to develop new and existing services.
  • Possess excellent communication, organizational and writing skills.
  • Ability to work with a diverse population of clients and community systems.
  • Knowledge of community resources.
  • Willingness to complete training regarding Elder Abuse and Hoarding
  • Follow all safety and PPE protocols as directed.

Minimum Education and Experience:

  • High School or GED completion.
  • Prior experience working with elderly community is preferred.
  • Ability to use Information Technology, including the internet, social media, Microsoft Office Suite products.

Special Conditions of Employment:

  • Valid Driver’s License, a working motor vehicle, and proof of current vehicle insurance.
  • Undergo and successfully pass a State of Michigan LIEN Check and Central Registry Clearance.
  • Pre-Employment Drug and Alcohol Screen.
  • Negative TB Test upon hire with follow-up TB tests as required by The Avenue policy.
  • Undergo a background check with no recent felony convictions and no abuse/neglect convictions. 
  • During On-Call phone rotation, must have the ability to work a flexible schedule including some evenings, weekends and holidays, as necessary.
