InterCare is a Federally Qualified Health Center which MAY qualify employed Behavioral Health employees for HRSA and PSLF student loan repayment programs!  *Eligibility may vary based on personal loan information*  

Starting Wages: LMSW: $73, 800 annually LLMSW: $57,600 annually and LPC: $65,707 annually.

InterCare is searching for a Behavioral Health Consultant to join our team at the Washington Street Elementary School in Otsego! Work Schedule:

Schedule varies; 24-hours/week

Minimum Requirements

  • Master's Degree in Social Work
  • LLMSW or LMSW licensure preferred
  • CAADC Certification preferred

Description of Primary Duties & Responsibilities

  • Provide consultation and assistance to medical providers on the management of behavioral health concerns in primary care and help implement the team-determined treatment plan.
  • Conduct brief screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing mental health or addiction treatment. Provide referrals for more intensive services where appropriate.
  • Provide short-term behavioral interventions to engage and motivate clients to actively manage their care. Conduct follow-up as appropriate based on assessment and treatment. Provide crisis management for clients as needed; make linkages for interventions as appropriate.
  • Participate in ongoing training in behavioral health issues and provide consultation and training to primary care clinic staff in mental health and behavioral medicine topics including communication skills, brief interventions, counseling skills, medication use and misuse, substance abuse, wellness, rehabilitation, recovery and self-sufficiency.
  • Functions effectively in a brief focused delivery model with integrated primary care teams.

Requirements

Professional and Technical Knowledge

  • Possesses a thorough understanding of the theory and practices of behavioral health integration, typically acquired through extensive practical experience in an integrated primary care environment and advanced learning through a master’s degree in Social Work with LLMSW or LMSW licensure preferred or doctoral level degree program.
  • Licensed Professional Counselor (LPC) consideration will be given based on previous behavioral health treatment experience.
  • CAADC Certification preferred, or expected to apply for CADDC development plan through MCBAP and obtain CAADC within years of employment.
  • Maintains a working knowledge of Joint Commission, PCMH, and IHI care coordination and integration standards.
  • Understands and consistently supports the uniform implementation of InterCare’s relevant clinical guidelines and organizational policies and procedures in assigned areas.
  • Focuses self and team members on functional outcomes for patients that improve quality of life and reduce unnecessary or inappropriate utilization of the health care system.

Physical Demands

Job duties are performed in the typical office environment of the organization, which requires ordinary ambulatory skills sufficient to visit other locations. The environmental factors and/or physical requirements of this position include the following:

  • Ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 lbs.
  • Requires good hand-eye coordination, reach with arms & hands, and finger dexterity, including ability to grasp, and manipulate (using keyboard, office equipment).
  • Visual acuity to use keyboard, operate office equipment, and read printed material and regularly required to talk and hear.
