Position Function Summary
The Behavioral Health Provider, a part of La Familia Medical Center’s (LFMC) primary care treatment team, identifies, triages and manages patients with behavioral health problems within the primary care setting. In addition, the Behavioral Health Provider will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patient’s behavioral health protocols for target populations.
Essential Responsibilities
- Assists primary health care providers in recognizing and treating mental disorders, substance abuse and psycho-social problems;
- Assesses the clinical status of patients referred by primary care providers through brief consultative contacts. Assesses, treats and refers pediatric, adults and geriatric patients, individuals, couples and families;
- Works with the primary care team to treat and manage patients with chronic emotional and/or behavioral health problems efficiently and effectively. Provides consultations for all clinic sites assigned;
- Assists in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration;
- Assists in preventing relapse or morbidity in conditions that tend to recur over time;
- Develops patient care plans with patient and behavioral health team;
- Teaches patients, families and direct care staff care, prevention, and treatment enhancement techniques, as well as family practice residents using BH curriculum;
- Supervises behavioral health interns;
- Notifies program directors of any identified problems related to patient services and makes recommendations for improvement;
- Attends and participates in meetings and quality improvement activities as required;
- Responds to patient or co-worker complaints and works toward a positive resolution of any dispute;
- Serves as a member of site committees, as requested;
- Attends residency IPOLS, planning meetings as requested by supervisor;
- Attends provider meetings at the request of supervisor.
Education, Experience and Skills/Training
- Master’s in Social Work (LCSW), or Master’s in Counseling Psychology (LPCC) required. Minimum three (3) years of experience in the field required.
- Current NM licensure as a Licensed Independent Clinical Social Worker (LCSW), Licensed Clinical Psychologist, Licensed Professional Clinical Counselor (LPCC), preferred. Licensed Master of Social Work (LMSW) or Licensed Mental Health Counselor (LMHC) (non-independently licensed) accepted. Supervision toward independent licensure level is required for this position, by clinical supervisor with licensure credential. Licensed Alcohol and Drug Abuse Counselor (LADAC) preferred.
- In addition, must possess substance abuse treatment/counseling experience, sufficient prior work experience in progressively more responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level. Experience in fast paced, clinic or hospital setting, working in counseling, psychology field with all age groups and serious mental illness.
- EMR experience;
- CPR Certification, required.
- Ability to make timely and accurate clinical assessments of mental and behavioral conditions;
- Good knowledge of psycho-pharmacology;
- Ability to design and implement clinical pathways and protocols for treatment of selected mental and behavioral conditions; to train and teach effective behavioral health interventions and primary behavioral health model to medical practitioners and other behavioral health providers;
