Client location and schedule hours:
Client located in the Franklin area. The schedule is Tuesdays, Saturdays and Sundays 8 am to 2 pm
Job Responsibilities:
Goals & Objectives: Keep hydrated, empty catheter as needed. Assist with lower leg compressions. Assist with all eating and drinking, preparing & serving. Light housekeeping (remove garbage @end of shift), laundry. bed linens. Engage with but allow him to sleep/rest as desired. Assist with showers or offer bed bath as desired.
ADL's: Mobility- Client is still mobile and can stand for 30 sec for transfers with assist. He can take several slow steps with his walker and CG always uses a gait belt to assist on transfers. He gets OT & PT 2x weekly.
Toileting: Client wears Depends all the time for leakage. Client can tell when he needs to have a bowel movement and with assistance, he makes it to the bathroom the majority of the time.
Eating: He has no food allergies. Cannot feel his hands, so cannot hold a cup to drink. He uses a straw and needs help, so CG should ask if he wants to drink about every 30min when he is awake. He needs assistance eating foods, with CG holding utensils.
Bathing/Dressing/Grooming: Client likes to get a shower 2-3x per week, With help, he can get into the shower and sit on the shower chair. We discussed eventual bed baths and he is good with that. Client does need help dressing due to numbness in his hands. We discussed the ability for CG to possibly help with bed baths when he is bedridden.
Communication: Client can speak and make his needs known. He is clear and articulate in his speech. He has a very polite and kind-hearted nature and is normally easy to get along with. He has no hearing loss at this point.
Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
