Job Description - Case Management Coordinator - 0.8 FTE


Prairie Ridge Health is looking for a team member to join our Patient & Family Services Department in the role of Case Management Coordinator. This position is a 0.8 (32 hours per week) Monday-Friday 7:00am-3:30pm. This position assumes responsibility and accountability to develop and coordinate case management and care effectiveness services within the hospital..... 

 POSITION SUMMARY

This person assumes responsibility and accountability to develop and coordinate case management and care effectiveness services within the hospital; provides leadership in the ongoing development of case management mission, goals, and functions which support the mission and goals of the organization; facilitates the development and implementation of case management processes; provides mentoring and coaching to the nursing and patient and family services department members; through the performance of daily chart reviews and other mechanisms gathers and facilitates the use of data for tracking, trending and evaluating outcomes; evaluates the need for change within the department in order to support the changing needs of the organization, and leads the change process.  Works collaboratively with nursing and medical staff to ensure that care decisions are value based and that care is provided in the most cost effective, and efficient fashion complying with evidence based practice, core measures, and regulatory standards of care.  Essential job duties do not present any exposure risks to blood, body fluids or tissue.

 EDUCATION REQUIREMENTS/LICENSURE/CERTIFICATION/REGISTRATION

  • Master’s degree (M.S, M.A. M.S.N.) or equivalent preferred, Bachelor’s Degree in Nursing (B.S.N.); or four to ten years related experience and/or training; or equivalent combination of education and experience required.
  • Current RN license, State of Wisconsin
