This part-time role offers flexible hours (typically between 9:00 AM – 4:00 PM) and averages 20–30 hours per week, making it a great fit for those seeking work-life balance while doing meaningful work.

At The Bridge Behavioral Health, we are a nonprofit organization dedicated to walking alongside individuals as they begin or continue their journey toward recovery. We offer a full continuum of care—including medically-monitored withdrawal, mental health and substance use respite, residential treatment, and outpatient services—all grounded in a trauma-informed, client-centered approach. As a Case Manager, you’ll be an important part of that mission—supporting clients as they stabilize, heal, and rebuild. You’ll help connect them to vital resources, provide individualized case management, and lead psycho-educational groups that equip them with the tools they need for long-term success.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Help foster and maintain a welcoming, trauma-informed, and safe environment for all clients.
- Establish rapport with peers and clients while maintaining professional boundaries
- Conduct and coordinate clinical case-management, including ongoing contact and coordination with probation, parole, diversion officers, case workers, therapists, family members, and/or any outside source involved with the client, in a manner that aligns with the clients’ individual treatment goals and objectives.
- Provide crisis intervention, as necessary, and the ongoing assessment of each client’s stability and functioning following a crisis event.
- Responsible for managing client referrals, referrals, orientation to treatment, continued stay and discharge process.
- Assist families with navigating the behavioral health system and accessing care for their loved ones.
- Follow up with clients who have returned home or with other informal supports as outlined in the discharge plan and offer additional supports as needed.
- Lead psychoeducational groups for clients in residential treatment.
- Complete timely documentation in the agency’s electronic health record (EHR).
- Participate in daily clinical team staffing and document staffing decisions.
- Attend and participate in monthly team meetings and quarterly all staff training as directed.
- Complete all monthly and annual online e-learning courses on time, in compliance with state and accrediting organizations training requirements.
- Assists with transportation when necessary.

MINIMUM REQUIREMENTS:
- Associate degree required, preferably in the Human Services or related field.
- Must be at least 21 years of age
- Must be eligible to work in the United States
REQUIRED KNOWLEDGE, SKILLS, ABILITIES:
- Ability to work within a team concept, establish and maintain professional boundaries with clients, and coordinate with other community agencies.
- Ability to communicate effectively both verbally and in writing.
- Initiative to identify needs and solutions while looking at problems analytically and solving them creatively.
- Excellent attention to detail and organizational skills.
- Knowledge of Microsoft Office including Outlook, Word, and Excel.
- Must have the ability to work with and relate to persons of multiple ethnic, racial, cultural and varied economic backgrounds.
DESIRABLE TRAINING AND EXPERIENCE:
- Bachelor’s degree
- Work experience in human services and/or case management
- Experience working with substance-dependent clients
- Bilingual encouraged to apply
