Case Manager - Adult Foster Care

About this role:

The AFC Case Manager provides home and community support for individuals with developmental disabilities, utilizing a team-based approach.  Promote individual growth in the areas of choice and decision-making, community membership, household management skills, relationships, and personal accomplishments.

Schedule: Monday, Wednesday, Friday - 24 hours/week
Start Date: 5/19/25

We are looking for people who have:

• Bachelor’s degree required.
• Valid Driver's License and reliable vehicle.
• Excellent organizational, interpersonal and communication skills.
• Experience in providing home-based supports required.
• Knowledge of AFC/Medicaid systems preferred.
• Commitment to community-based supports to individuals with developmental disabilities.
• Able to meet the required competency level in Microsoft Office Software.

In this role you'll get to:

• Facilitate and coordinate home placement and support services for individuals based upon individual needs and preferences.
• Coordinate referral, intake and placement process.
• In collaboration with Program Supervisor, function as liaison with funding agencies.
• Ensure completion and maintenance of documentation as required by KDC and all applicable funding agencies.
• Participate in professional development activities and training’s necessary to enhance skills and maintain licensure.
• Data entry and tracking within the Electronic Healthcare Record (ICENTRIX)
• Uploading required information in ICENTRIX
• Finalizing and closing documents within ICENTRIX
• Promote individual choice and self-expression based upon quality service principles.
• Assist in the development of the Individual Care plan/ Individual Support Plan.
• Work cooperatively/collaboratively with other community service providers and medical professionals to ensure coordination of supports and services.
• Participate in staff meetings, group and individual supervision, and case reviews.
• Participate in the planning process to ensure adequate support in the areas of health and safety.
• Assist in outreach activities and program fund-raising activities
• Extensive travel is required for this role with mileage reimbursement.
• Perform other job-related responsibilities as assigned.
