Your Role as a CNA at Heart To Serve Home Care LLC
As a Certified Nursing Assistant (CNA), you are the heart and hands of our home care services. Your work ensures that clients live safely, comfortably, and with dignity in their own homes. Responsibilities Include:
Personal Care Assistance- Provide respectful and professional bathing, dressing, grooming, toileting, and hygiene support.
Safe Transfers & Mobility Assistance- Help clients ambulate safely, prevent falls, and transfer from bed to wheelchair.
Emotional & Social Support- Be a companion, a listener, and a source of comfort to clients.
Monitoring & Reporting- Observe any changes in condition and report to the supervising RN immediately.
Meal Preparation & Nutrition Support- Plan and prepare nutritious meals according to dietary needs.
Medication Reminders- Ensure clients take medications on schedule as prescribed.
- Light Housekeeping & Home Safety-Maintain a clean, organized, and hazard-free environment.
Dementia & Alzheimer’s Care- Use patience, redirection techniques, and structure to support memory care clients.
Family Communication- Provide updates and reassurance to client’s families when appropriate.
Documentation & Compliance- Keep accurate daily care logs and adhere to HIPAA confidentiality guidelines.
We are seeking CNA professionals who:
- Have a genuine passion for serving seniors and improving their quality of life.

- Demonstrate reliability, integrity, and professionalism at all times.
- Have strong communication skills and the ability to work independently.
- Are able to problem-solve and respond effectively to client needs.

Minimum Requirements (Pre-Screening Required):
- To qualify for this position, you must have:
- Active CNA license in the state of Indiana
- Government-issued ID (Drivers License or State ID)
- Social Security card or birth certificate
- Current CPR certification (Required)
- Negative TB test or chest X-ray (Required for client safety)
- Criminal Background Check
- Reliable transportation (Ability to travel to client homes)
