Summary:
Provides in-home personal care and support to clients under the supervision with the collaboration of the Care Team Manager and Client Care Manager Assists with daily living needs, monitors basic health indicators, and helps maintain a safe, comfortable home environment.

Responsibilities:

  Assist with bathing, dressing, grooming, toileting, and feeding

  
  


• 
  

  Support mobility: transfers, ambulation, and exercises

  
  


• 
  

  Take and record vital signs and report changes in condition

  
  


• 
  

  Provide companionship and emotional support

  
  


• 
  

  Light housekeeping and simple meal preparation

  
  

Qualifications:

• 
  

  Active state CNA certification REQUIRED

  
  


• 
  

  1 year experience

  
  


• 
  

  Reliable transportation

  
  


• 
  

  Strong communication and companionship
