POSITION TITLE:  Community Health Worker

RESPONSIBLE TO: Community Health & Engagement Lead

HRS PER WK: 10 hours/week (hybrid position based in Eugene, OR)

CLASS: Non-exempt 

SALARY: $20 - 23.75,  Depending on experience [+$1 bilingual differential for Spanish fluency] 

BENEFITS: Eligible for FFLC sponsored benefits (e.g. paid time off benefits such as vacation, sick, holidays) and eligible to participate in 401k retirement plan. 

Duration: This role is expected to last for a period of 1 year and may be extended after evaluation at the 1-year period. 

NOTE FOR INTERNAL APPLICANTS: This position may be combined with an existing part-time position pending current supervisor approval

ABOUT FOOD FOR LANE COUNTY: The mission of FOOD For Lane County (FFLC) is to reduce hunger by engaging our community to create access to food.  We accomplish this by soliciting, collecting, rescuing, growing, preparing and packaging food for distribution through a network of more than 150 social service agencies and programs; through public awareness, education and community advocacy; and through programs designed to improve the ability of low-income individuals to maintain an adequate supply of wholesome, nutrition food. We serve a diverse population of people living on limited incomes including children, families, seniors, and single adults.   

PRIMARY TASKS & RESPONSIBILITIES:

  • Provide resource referrals and screenings to community members based on their eligibility and interest in available food assistance supports.
  • Provide referrals and education for food assistance and other appropriate programs
  • Support participants’ engagement with nutrition education curriculum by gauging interest, following up, scheduling, etc.
  • Meaningfully engage with community members at local community engagement events
  • Represent FFLC at tabling events, presentations, and other opportunities to provide resources to community members
  • Receive training and provide nutrition education to community members as appropriate
  • Provide housing navigation, employment navigation, food assistance navigation, case management, and other services, including referrals, as appropriate
  • Complete health-related social needs screenings with presumed eligible clients in their preferred language (English or Spanish)
  • Assess client fit for nutrition programs
  • Utilize the UniteUs platform to complete screenings and make referrals to internal and external programs, partners, and community-based organizations
  • Follow processes to provide eligible clients with nutrition services and offer relevant referrals as needed
  • Ensure appropriate billing and reimbursement by accurately reporting provided services and referrals
  • Collect and store relevant client data appropriately (including personal health information) in alignment with HIPAA regulations and responsibilities 

ADDITIONAL TASKS:

  • This role may include supporting nutrition education by gathering ingredients, preparing simple recipes, and demonstrating the cooking process for program participants.
  • Coordinate closely with Supervisor to ensure appropriate workloads and adequate coverage of assigned tasks
  • Actively demonstrate FFLC’s values of compassion, collaboration, and inclusion
  • Represent FFLC to the public with knowledge and respect
  • Attend mandatory All staff meetings
  • Represent FFLC to the public with knowledge and respect
  • Adhere to applicable FFLC Policies and Procedures
  • Perform other duties as assigned by supervisor

SKILLS and QUALIFICATIONS: We value all forms of experience including unpaid work, lived experience, education, and training.

  • Community Health Worker trained and certified (or able to become certified)
  • Exceptional phone and customer service skills
  • Familiarity with UniteUs or other case management systems is a plus
  • Ability to work with people from a wide diversity of backgrounds
  • Professional, thoughtful, and respectful verbal communication skills
  • Friendly, compassionate, and respectful attitude
  • Receptivity to feedback, training, and direction
  • Maintain confidentiality respective with HIPAA laws when handling personal health information
  • Possess a current Food Handler’s Card or be willing to obtain one.
  • Respond within 3 business days to emails, text messages, or phone calls
  • Sensitivity to, and ability to maintain, confidential information
  • Empathetic, compassionate, and understanding of daily stressors facing people with low/no-income
  • Reliable access to the internet and a device with video capabilities
  • Ability to occasionally use your own car to attend events as directed by a supervisor (Mileage is paid)
  • English/Spanish bilingual required – Spanish proficiency will be assessed to qualify for differential pay
