Direct Support Coordinator

Harvest Healthcare Solutions is seeking qualified, dedicated Part-Time Direct Support Coordinators  in South Lake Tahoe. This position provides support to individuals with developmental disabilities in their family homes, with the goal of promoting safety, independence, and the development of essential life skills.

Direct Support Coordinators deliver person-centered services, coordinate supports, assist with daily living skills, and advocate for individual needs in accordance with Title 17 requirements.

Minimum Qualifications:

• Experienced supporting individuals in developing independence, life skills, and community involvement


• Demonstrated interest in supporting individuals with developmental disabilities


• Ability to provide individualized, person-centered services


• Minimum of 1–3 years of experience working with individuals with disabilities or in a human services setting


• Ensure health, safety, and well-being of individuals served at all times

Language proficiency is required to ensure effective communication with the individuals and families served.

Salary Range:  $17.50 - $19.50 per hour (Dependent on Experience)

Primary Responsibilities:

1. Individualized Support & Skill Development:
   
   
   
   • Teach daily living skills such as hygiene, meal preparation, and money management.
   
   
   • Provide coaching in social skills and self-advocacy training.
   
   
   • Support the use of assistive technology and adaptive devices.
   
   
   
   


2. Community Integration & Resource Access:
   
   
   
   • Facilitate community participation and help individuals navigate transit.
   
   
   • Assist individuals in accessing employment, education, and volunteer opportunities.
   
   
   • Help connect individuals to necessary benefits and health services.
   
   
   
   


3. Service Coordination & Case Management:
   
   
   
   • Collaborate with regional centers, service providers, and families.
   
   
   • Support the implementation of Individual Program Plans (IPPs) and service authorizations.
   
   
   • Maintain detailed and timely documentation of services and activities.
   
   
   
   


4. Crisis Support & Problem-Solving:
   
   
   
   • Provide behavioral and emotional support as needed.
   
   
   • Assist with emergency preparedness and safety planning.
   
   
   • Liaise with crisis teams and mental health providers to address immediate needs.
   
   
   
   

Qualifications & Requirements:

• Education:
  
  
  
  • High school diploma or GED required.
  
  
  • AA or BA in Human Services, Psychology, or a related field preferred.
  
  
  
  


• Experience:
  
  
  
  • Minimum 1 year of experience supporting individuals with developmental disabilities.
  
  
  
  


• Language Skills:
  
  
  
  • Bilingual (English required, and proficiency in Spanish, Tagalog, or Russian).
  
  
  
  


• Skills:
  
  
  
  • Strong interpersonal and communication skills.
  
  
  • Knowledge of community resources and service providers.
  
  
  • Proficiency in documentation and maintaining accurate records.
  
  
  
  


• Certifications:
  
  
  
  • CPR/First Aid required or to be obtained within 30 days of hire.
  
  
  
  


• Other Requirements:
  
  
  
  • Valid driver’s license and reliable transportation.
  
  
  • Clear background checks, drug screening, and TB test.
