About the Role:
As a Direct Support Professional at HealthChoice Waiver Services, you will play a vital role in empowering individuals with disabilities, developmental challenges, or special needs to lead independent and fulfilling lives. You will provide hands-on support, encouragement, and assistance in daily living activities while fostering a safe, respectful, and engaging environment.
Key Responsibilities:
- Assist individuals with daily living tasks, including personal care, hygiene, meal preparation, and mobility support.
- Provide emotional support and companionship while promoting independence and self-confidence.
- Support individuals in community integration, such as outings, social events, and recreational activities.
- Help develop and implement individualized care plans to achieve personal goals and improve quality of life.
- Document daily activities, progress, and any concerns to ensure the highest level of care.
- Communicate effectively with team members, families, and healthcare professionals.
Qualifications & Skills:
- Experience in caregiving, healthcare, or social services is a plus but not required—training is provided!
- Compassionate, patient, and dedicated to making a positive impact.
- Ability to lift, assist, or provide physical support as needed.
- Strong communication and teamwork skills.
- Valid driver’s license.
