Director of Nursing (Immediate Start Interim to Permanent)

Job Summary
Actual HomeCare is seeking an experienced and dedicated Director of Nursing (DON) to lead our clinical functions during a transitional period. This role begins as an interim or part-time opportunity (20–30 hours/week) with the potential to become full-time and permanent based on organizational needs.

Responsibilities as Director of Nursing (DON):

• Provide clinical leadership and supervision to nursing staff and caregivers.


• Develop, implement, and monitor policies and procedures in compliance with federal, state, and local regulations.


• Support CHAP survey readiness, audits, and documentation (with consultation as needed).


• Conduct initial assessments and re-assessments, and ensure care plans are accurate, comprehensive, and up to date.


• Interviewing caregiver staff, training and performance evaluations.


• Collaborate with families, and work closely with the executive team to ensure client satisfaction.


• Monitor quality assurance, infection control, and patient safety initiatives.


• Provide mentorship and professional development opportunities for the care team.

Requirements:

• Active Registered Nurse (RN) License in the state of New Jersey.


• Minimum of 2+ years supervisory or leadership experience (home care preferred).


• CHAP experience preferred but not required, willingness to learn or work with consultation acceptable


• Strong knowledge of home care regulations, compliance, and best practices.


• Excellent leadership, communication, and organizational skills.


• Willingness to travel up to one hour to conduct patient visits.


• Ability to work flexible hours and weekends as needed.
