SUMMARY
The Family Care Giver (FCG) performs varied and diverse work and is responsible for person-centered service to PDI people served. The FCG supervises and supports a person served as outlined in the person’s Individual Service Plan and is approved for same by the Community Centered Board representative and Colorado Medicaid Services. PDI, as employer for the FCG, maintains oversight of the services the FCG provides. The FGC documents and implements individual service and support plans. The FCG maintains close contact with appropriate PDI staff regarding individual needs and requirements. The FCG works to ensure a safe and healthy environment for each assigned individual served. As an FCG, he or she may provide any of the following services as outlined in the Individualized Service Plan.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Individual Care and Support
· Personal Care
o Assure that people served receive adequate and skillful support with activities of daily living (“ADLs”) as required, including:
o Basic ADLs
o Instrumental ADLs
o Provide one-to-one assistance to people served with lavatory functions and will change and clean people being served promptly when soiling occurs
o Ensure safe transfer, lift usage and properly position and belt people served in wheelchairs
o Observe closely that the people served, and their clothing is clean and dry

Requirements
MINIMUM REQUIREMENTS FOR ALL PDI POSITIONS
You must be age 21 or over to work at PDI. You must also pass a pre-hire drug and background screen as well as random drug screens that may occur at any time and without notice while employed by PDI. You must be computer-literate, comfortable with technology with well-developed oral and written communication skills. You must be able to complete and pass First Aid, CPR and other required training.
If the position requires you to drive a PDI vehicle on company business, then you must possess a valid driver’s license and agree to a review of your motor vehicle record for addition to PDI’s authorized drivers list. In addition, if the position authorizes you to drive your vehicle on PDI business then you must provide proof of automobile insurance, current and valid driver's license and driving record.
REQUIRED EDUCATION AND EXPERIENCE
High school diploma or general education degree (“GED”) required; experience working with individuals with disability preferred.
LANGUAGE ABILITY
Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to clearly and legibly write routine reports and correspondence. Ability to speak effectively with people served and their support networks and co-workers.
MATHEMATICS ABILITY
Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.
COMPUTER SKILLS
Knowledge of word processing software, spreadsheet software and Internet software. Ability to learn and gain proficiency in other software and applications used by PDI.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several variables.
POSITION TYPE AND EXPECTED HOURS OF WORK
This is a non-exempt full-time or part-time position based on the FCG’s Individualized Service Plan assignment.
PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable persons with disabilities to perform essential functions.
The FCG must regularly lift and /or move up to 50 pounds, frequently lift and/or move up to 100 pounds and occasionally lift and/or move more than 100 pounds with the aid of an assistive device. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. While performing the duties of this Job, the FCG is regularly required to reach with hands and arms and talk or hear. The FCG is frequently required to stand; walk; sit; use hands to finger, handle, or feel; climb or balance and stoop, kneel, crouch, or crawl. The FCG is occasionally required to taste or smell.
