About the Role:

Join Tradition Home Health Care as a Home Health Aide (HHA-PCA) in Brooklyn, NY, where you will make a meaningful impact in the lives of our clients. We are seeking compassionate and dedicated individuals to provide essential personal care and support in a nurturing environment.

Responsibilities:

Assist clients with daily living activities such as bathing, grooming, and meal preparation.

Provide companionship and emotional support to enhance clients' quality of life.

Monitor and report changes in clients' health and well-being to the care team.

Administer medication and assist with mobility as needed.

Maintain a clean and safe living environment for clients.

Document care provided and maintain accurate records in compliance with company policies.

Collaborate with healthcare professionals to ensure comprehensive client care.

Participate in training and development opportunities to enhance skills and knowledge.

Requirements:

Valid Home Health Aide (HHA) or Personal Care Aide (PCA) certification.

Experience in home health care or a related field preferred.

Strong communication skills and a compassionate demeanor.

Ability to perform physical tasks and assist with mobility.

Reliable transportation or a valid driver's license.

Willingness to work flexible hours, including evenings and weekends.

Background check and references required.

First aid and CPR certification is a plus.
