For your convenience, we will provide you with a list of institutions and agencies that can administer TB Tests/Chest X-rays or conduct CPR and first Aid classes. Please ask our Human Resources representative for a copy of this list.
Responsibilities
Visit patients in their homes for one-on-one visits
Complete initial and ongoing assessments to identify the physical, psycho-social, and environmental needs of home health patients
Assess patients’ conditions and chart observations, and perform evaluation tasks, including vital signs and medication review
Administer medication as prescribed by the patient’s physician
Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
Provide effective communication to patients/clients, their family members, team members, and other health care professionals
- Assessment & Planning: Conduct comprehensive patient assessments, collect health information, interpret data to establish diagnoses, and formulate detailed care plans.
- Medication & Treatments: Administer medications and treatments as prescribed by physicians.
- Patient and Family Education: Educate patients, families, and caregivers on disease processes, treatment methods, and health promotion strategies.
- Monitoring & Evaluation: Observe and monitor patient conditions, including changes in physical or emotional health, and evaluate their response to care, making necessary adjustments to the care plan.
- Coordination: Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure the timely provision of health services.
- Supervision & Delegation: Monitor and direct Licensed Practical Nurses (LPNs) and other care staff in performing delegated nursing tasks.
- Documentation: Accurately document patient care, observations, interventions, and progress according to agency policies and procedures.
- Infection Control & Wound Care: Provide specialized wound care and implement strategies for infection control
Qualifications
- Proof of Right to Work in the United States
- Valid Driver's License or state-issued ID Card
- Proof of Automobile Insurance
- CPR / First Aid Card (online cards will not be accepted)
- Negative TB Test; if positive, a negative reading of the Chest X–Ray
- Criminal Background Check
- Three (3) verifiable References
Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
Current state license as an RN
Proof of current CPR certification
Compassionate, friendly demeanor
Excellent communication skills
Current driver’s license and reliable transportation
Ability to work a flexible schedule and travel locally
