Home Care Assessment RN

The job requires an initial assessment for each all age client/children , 30 day, and 90 day on going supervisory visits.  The visits will include: consultation, information gathering while evaluating the client's medical history, physical condition, and specific ADL healthcare needs. Maintain and keep accurate and comprehensive records of assessment findings, care plans, and progress notes.  
 RN will need to sign off on weekly employee time-sheets -at their scheduled time and return them to be filed. 

Each visit last approximately 1 hour at an hourly rate of $35/hr and time/day appointments are based on your schedule, also paid $.57 per mile gas .
