Job Summary

The Care Coordinator takes an active role as part of the patient care team to facilitate excellent communication, quality care, and support to both clinicians and patients.

Key Responsibilities

• Coordinate visits with the care team and patient


• Ensures all patient schedules are following MD orders


• Verify insurance


• Help facilitate communication between care team members


• Communicate with physicians to ensure orders are signed

Job Types: Full-time, Part-time

Pay: $18.00 - $20.00 per hour

Expected hours: 20 – 25 per week

Schedule:

• Monday to Friday

Ability to Commute:

• Cedar City (Required)
