Job Description - Home Health- Licensed Practical Nurse (Full Time, Part Time, Per Diem)



Job description:






We are seeking CT LPN's to join our team!


We work throughout Fairfield County in CT


Responsibilities:



  • Administer nursing care at homes.

  • Establish a Plan of Care and treatment plans

  • Educate patients about health maintenance and disease prevention

  • Provide advice and emotional support to patients and their family members


Qualifications:



  • Licensed in Connecticut

  • Previous experience in nursing

  • 2 years of home care experience

  • Ability to build rapport with patients
