Responsibilities:

• Provide high-quality, individualized nursing care to patients in their homes under the supervision of a Registered Nurse and as directed by the physician’s plan of care.
• Assist in implementing the patient’s care plan in collaboration with the RN, physician, and other members of the healthcare team.
• Observe and report changes in the patient’s condition to the RN case manager, physician, and other team members as appropriate.
• Administer medications and treatments as prescribed and within the scope of LVN practice.
• Participate in on-call rotation (approximately every 3–4 weeks) and perform PRN visits as needed.
• Accurately document care and patient interactions using the Kinnser (WellSky) system.
• Support patient and family education as directed by the RN or care team.

Requirements:

• Current LVN license in the state of Texas
• Minimum 1 year of LVN experience (Home Health preferred)
• Proficiency in English (spoken and written)
• Valid Texas driver’s license and current auto liability insurance
• Current CPR/BLS certification
• Familiarity with Kinnser/WellSky is a plus
