Are you a compassionate and reliable Licensed Practical Nurse (LPN) looking to make a real difference in your community? Patient Centered Services, LLC is currently seeking dedicated Home Health LPNs to join our growing team in Northern Virginia!
Position Type: Part-Time & Full-Time
Pay Rate: $45/hour
Location: Northern Virginia (assignments based on your availability and location preference)

Responsibilities:
- Provide skilled nursing care to clients in their homes under a physician’s plan of care
- Monitor client health, document observations, and report changes
- Administer medications and treatments as prescribed
- Educate clients and family members on care and wellness
- Collaborate with interdisciplinary team members to ensure continuity of care
Requirements:
- Current LPN or RN license in the state of Virginia
- Minimum of 1 year of nursing experience (home health preferred but not required)
- CPR Certification
- Reliable transportation and valid driver’s license
- Strong communication, time management, and documentation skills
- Must pass background check and drug screening
