We are seeking an experienced and compassionate Part-Time/Per Diem Physical Therapist (PT) to join our home health care team. As a PT, you will provide individualized physical therapy services to patients in their homes, helping them regain mobility, strength, and independence after illness, injury, or surgery. You will collaborate with a multidisciplinary team to ensure that each patient receives personalized care and achieves their rehabilitation goals.
Key Responsibilities:
- Perform comprehensive patient evaluations to assess mobility, strength, balance, and functional limitations.
- Develop and implement individualized treatment plans to improve physical function, mobility, and overall quality of life.
- Provide hands-on therapy, including therapeutic exercises, manual techniques, and other interventions to restore movement and function.
- Educate patients and their families on safe mobility techniques, home exercises, and strategies to prevent injury or complications.
- Monitor and document patient progress, adjusting treatment plans as necessary to meet therapy goals.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate and timely documentation of patient evaluations, treatments, and progress using the electronic medical record (EMR) system.
- Ensure compliance with agency policies, procedures, and state/federal regulations for home health care.
Qualifications:
- Active and unrestricted Physical Therapist (PT) license in [State].
- Minimum of 1 year of clinical experience, preferably in home health, rehabilitation, or a related field.
- Strong knowledge of physical therapy principles and home health care regulations.
- Proficiency in electronic medical record (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a diverse patient caseload effectively.

- Valid driver’s license and reliable transportation.
