Job Description - Home Health - Physical Therapy Assistant

HOME HEALTH – PHYSICAL THERAPIST (RPT)

We have a great opportunity for a Part Time Home Health Physical Therapy Assistant in Muncie / Alexandria, Indiana area.

1:1 Patient Care

Service Area – Muncie / Alexandria

Job Responsibilities

The Physical Therapy Assistant is responsible for implementing programs that focus on rehabilitation.
Work directly with patients on therapeutic exercises designed to help regain full mobility, strengthen their muscles, and improve coordination, following plan of care specifically written for individual patient.
Observe and reassess patient's progress.
Coordinate treatment plan with patient, family and other team members.
Collaborate with PT and other disciplines to ensure best patient outcomes.
Evaluate effects of therapy treatment and communicate patient's progress.
Ensure patients receive personalized care.
Provide training for using assistive devices, such as crutches, canes or prosthetics.
Record and document patient care services.

Education, Licensure(s), And Qualifications.

Valid Physical Therapist Assistant licensure as required by state regulations
CPR certification required (ACLS preferred)
Previous Home Health experience is preferred but not required.
Demonstrates good verbal and written communication and organizational skills.
Must possess attention to detail skills.
Ability to prioritize work using fundamental time management skills.
Must possess excellent interpersonal skills to foster a team environment.
Demonstrates knowledge and technical skills necessary for providing competent, comprehensive patient care.
Must possess problem solving abilities, excellent customer service skills, and patient first characteristics.
Valid driver's license, valid auto insurance and reliable transportation required
Valid TB test (or have one placed in our office)
