Position Overview

Reporting to the VP of Home Care and Hospice, the Hospice House Clinical Supervisor supports the clinical operations of the Hospice House and Hospice Community on the weekends. The Clinical Supervisor manages by planning and prioritizing, organizing work, keeping daily operations functioning, setting attainable goals, and makes decisions based on the mission and services of the Agency. The Clinical Supervisor will supervise Registered Nurses, Licensed Practical Nurses, and Licensed Nursing Assistants at the Hospice House and in the community, as well as other Hospice House team members as assigned. The Clinical Supervisor is also responsible for delivering patient care as needed.

Essential Functions

  • Provides clinical supervision oversight to nursing staff at the Hospice House and in the community.
  • Supports initiatives established by the QAPI program, performance improvement projects and medical review.
  • Coordinates Hospice House and community patient referrals and patient care with the Medical Director, attending providers and hospice leader, ensuring that patient needs are met and continuously assessed according to the individual needs of the patient and physician ordered Plan of Care.
  • Assures compliance with all state, federal laws regarding licensure and certification of the agency. Maintain knowledge of the Medicare Conditions of Participation for Hospice and an understanding of NH Hospice Administrative Rules.
  • Facilitates IDG meetings with staff, as appropriate, to ensure that multi-disciplinary cases plan of care, goals and discharge plans are aligned and coordinated between team members.
  • Monitors clinical documentation to insure compliance with agency policy and procedures, regulatory and accreditation requirements.
  • Interviews and recommends new candidates for hire.
  • Coordinates supervisory visits of nursing staff in accordance with agency policy and procedure.
  • In conjunction with the team member’s manager, completes performance evaluations for direct reports at the end of the introductory period and annually.
  • Participates in identifying and planning for nursing and aide clinical competencies.
  • Ensures that competencies and required education is completed timely.
  • Addresses scheduling needs on the weekend to ensure adequate nursing staffing for the Hospice House and community.
  • Receives and documents reports of incidents, complaints, and grievances according to Policy and Procedure.
  • Supervises Registered Nurses, Licensed Practical Nurses, and Licensed Nursing Assistants assigned to patients according to federal and state regulations.
  • Orients, coaches, mentors and precepts new nursing team members.
  • Covers nursing shifts as needed for staffing gaps and shortages as needed.
  • Provides on call coverage from Friday 5pm through Monday at 7am.

Other Job Functions

  • Builds and maintains positive relationships with our customers (clients, applicants, staff member and managers) by readily responding to their questions and concerns.
  • Provides the Paraprofessional Nurse Educator with input for in-service education needs of the program.
  • Documents accurate and ongoing assessment/reassessment, treatment and discharge of the patient: nursing interventions, patient response to care provided, patient/caregiver educational needs, problems, capabilities, limitations and progress toward goals. Documentation includes evidence of appropriate patient/caregiver teaching, and the understanding of these instructions in accordance with agency guidelines
  • Engages the patient /caregiver in meaningful education as indicated per care plan.
  • Demonstrates responsibility to effectively coordinate the patient’s POC with members of the Interdisciplinary Team and other health professionals through team/patient care conference. Updates patient’s short-term/long-term goals as appropriate based on interdisciplinary team assessments and re-assessments.
  • Attends Agency In-service programs.
  • Completes educational requirement to maintain clinical licensure and skills.
  • Demonstrates flexibility in the role by accepting other duties as assigned for which qualified.
  • Performs job duties in a safe manner in order to prevent injury to yourself and others

Expected Hours of Work

The team member will be scheduled to work Friday-Sunday: 7 am – 3:00 pm with the opportunity to pick up other shifts as an RN when available. Health and welfare benefits offered to all team members who work over 20 hours/week. Other hours, as well as holiday work may be required based on Agency needs.


Requirements

  1. Holds a current New Hampshire RN license, or compact state license through the Nurse Licensure Compact (NLC)
  2. Three to four years of recent registered nurse experience with two years in a hospice or palliative care setting
  3. One to two years of supervisory experience or relevant work experience
